7. Renewal of District Health Insurance
Recommended Motion: that the Board of Education approve the renewal for the administration of the District Health Insurance for the calendar year 2017 with Blue Cross Blue Shield of Illinois Insurance Company with Administrative costs for all active employees and retirees being $37.11 per employee per month and Individual Stop Loss fees for all active employees and retirees under age 65 being $55.02 per employee per month. 8. Renewal of ASO Dental, Group Term Life and AD&D, Dependent Life Insurance, Voluntary Life Insurance and Long Term Disability Insurance Recommended Motion: that the Board of Education approve the renewal for ASO Dental, Group Term Life and AD&D, Dependent Life Insurance, Voluntary Life Insurance, and Long Term Disability Insurance with The Guardian Life Insurance Company for the 2017 calendar year. See Attachment No. 3.
9. Renewal of Voluntary Vision Insurance
Recommended Motion: that the Board of Education approve the renewal for Voluntary Vision Insurance through United Healthcare for the 2017 calendar year with a 3 year rate guarantee. See Attachment No. 4.
