A nursing home in Rock Island County and another in Whiteside County have been cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the second quarter of 2017.
The report, released on Monday, lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively. Both of the facilities received A violations and $25,000 fines.
Hope Creek Care Center was cited with an “A” violation and fined $25,000 for a failure to safely transfer a resident, resulting in a broken femur. The 245-bed skilled care facility at 4343 Kennedy Drive, East Moline, waived its right to a hearing and paid $16,250.

Pleasant View Rehab and Health Care Center, a 74-bed skilled care facility at 500 N. Jackson St., Morrison, was cited with an “A” violation and fined $25,000 for failing to prevent a resident from falling from a chair and suffering several fractures. The facility requested a hearing.
